Before you order, please note:

• Turnaround Time: Please allow 7–10 business days for processing and standard shipping.*
• Shipping Addresses: We cannot deliver bulk shipments to P.O. Boxes; please provide a physical street address for each clinic.
• Multiple Clinics: You can request shipments for up to 5 different locations using this form.*
• Quantity: Our brochures are shrink-wrapped in bundles of 25. The maximum order quantity is 500 brochures per shipping address (20 shrink-wrapped bundles of 25 brochures).*

*If you would like to request brochures for delivery to more than 5 locations, a quantity that exceeds, and/or need to be delivered sooner than 7 business days from today, please contact our Customer Support team.
